It has been given many names, including patellofemoral disorder, patellar malalignment, runner's knee, and chondromalacia. Patellofemoral pain syndrome can occur from overuse of the knee in sports and activities such as running, walking, jumping, or bicycling. The kneecap (patella) is attached to the large group of muscles in the thigh called the quadriceps. It is also attached to the shin bone by the patellar tendon. The kneecap fits into grooves in the end of the thigh bone (femur) called the femoral condyle. With repeated bending and straightening of the knee, you can irritate the inside surface of the kneecap and cause pain. Patellofemoral pain syndrome also may result from the way your hips, legs, knees, or feet are aligned. This alignment problem can be caused by your having wide hips or underdeveloped thigh muscles, being knock-kneed, or having feet with arches that collapse when walking or running (a condition called over-pronation).

Anterior Thigh/knee Pain Case Query
My feeling is that this patient may have a low grade inflammation of the left kneejoint as a result of a patellofemoral disorder, with subsequent swelling.

Extractions: buring/numbness in an area including the lateral aspect of the left knee and inferolateral thigh, and superior to the patella. From an extended position, the area extends 10cm superiorly from the patella and spreads superolaterally. The 'pain' is rated as 8/10. The pain feels like it is in the skin - "like a rash or cut". The patient also has objective knee swelling, especially obvious above the knee, with a positive patella tap sign and has a history of lateral dislocation at age 13. The current episode came on 4 months ago for no apparent reason. x-rays and ultrasound were NAD. The pain now comes on at rest, and prevents the patient from walking to the shop. Every now and then the patient also feeling a stabbing, shooting pain in the same distribution as described above. On examination, all orthopaedic tests of the knee were negative, however the patient was quite apprehensive when moving the patella and said 'it feels like it is going to pop out!' The patella is highly mobile and sits quite high on the femur. Specific tests of patella movement were equivocal, due partly to the discomfort they caused the patient. There is no doubt though that there is something going on locally, in the knee joint. However, the burning/numb feeling does not sound typical, and lightly squeezing the skin above the knee and on the lateral/inferior aspect of the thigh reproduces the patients burning pain. Sensory testing reveals a loss of light touch discrimination in the pain distribution. Also, the ability to discriminate between sharp and dull is lost in the same area. The right knee is normal, with no alteration in sensory function.
