Besides being cosmetically distressing, hirsutism may also signal the presence of a hormone imbalance or a hormone-producing tumor. Normal hair growth Each hair grows from a follicle deep in the skin. As long as these follicles are not completely destroyed, hair will continue to grow even if the shaft, which is the part of the hair that appears above the skin, is plucked or removed. Adults have two types of hair, vellus and terminal. Vellus hair is soft, fine, colorless, and usually short. In most women, vellus hairs grow on the face, chest, and back and give the impression of "hairless" skin. Terminal hairs are the longer, coarser, darker, and sometimes curly hairs that grows on the scalp, pubic, and armpit areas in both adult men and women. The facial and body hair in men is mostly of the terminal type. What causes hirsutism? Most often, excess facial and body hair is the result of abnormally high levels of androgens or male hormones in the blood. Androgens are present in both men and women, but men have much higher levels. These hormones cause hairs to change from vellus to terminal. Once a vellus hair has been transformed to the coarser terminal hair, it usually does not change back. Androgens also cause terminal hairs to grow faster and thicker. Both the ovaries and the adrenals produce androgens. To some degree, estrogens and progesterone, female hormones, prevent the effect of androgens.

These hormones are androstenedione, dehydroepiandrosterone sulfate, and testosterone. Androstenedione is made in about equal amounts by the ovaries and the adrenal glands. Dehydroepiandrosterone sulfate (DHEAS) is secreted by the adrenal glands. 60 % of the testosterone in the body is made by the ovaries, while 40% is made when androstenedione in the blood is converted to testosterone. Testosterone is the active hormone (the other two act as precursors, but do not have a direct effects by themselves).

Extractions: This is usually due to above average levels of androgens, the male hormones that are normally present in women at low levels. Some women do not find the excess hair a problem, particularly if it does not affect their face, or it is blonde in colour. Sometimes excess hairiness is not abnormal and is a racial or genetic variation. Initial treatments include bleaching and electrolysis. If these do not produce an acceptable result, drugs may be used to reduce the high androgen levels, if that is the cause. The contraceptive pill contains oestrogen , which reduces androgen levels and will improve hirsutism. A formulation is available which includes a specific drug to reduce these further, known as 'Dianette'. The other component of Dianette is called cyproterone acetate, and this is the next drug to try if hirsutism persists. It is used in a higher dose than contained in the Dianette pill, but must be combined with adequate contraception, as it can cause fetal abnormality if taken during early pregnancy. Spironolactone is another alternative, but this frequently causes erratic periods, so is often given with a low dose contraceptive pill. A newer drug is called flutamide , which appears promising, though its safety profile is less clear. Side effects of the anti-androgens include tiredness, mood changes and reduced sex drive. Both flutamide and higher dose cyproterone acetate have a rare but serious side effect of causing problems with liver function, and so regular blood tests are advised.

At least 25% of normal middle-aged women have unwanted facial hair and many women do not realise the treatment options that are available. Women have the same number of hairs on their bodies and faces as men do, but the male hormone, testosterone, makes hairs thicker, darker and longer, making them more noticeable. Some women with noticeable body hair have normal amounts of testosterone and inherit this tendency. Others have a tumor or medical condition that causes their bodies to produce larger amounts of testosterone. Women with noticeable body and facial hair should get blood tests for testosterone to measure male hormones made by the ovaries and DHEAS to measure male hormones produced by the adrenal glands. They should also get a sonogram of their ovaries to test for polycystic ovaries (PCO syndrome) in which the ovaries do not release eggs each month. PCO syndrome is treated like diabetes with drugs (Glucophage, Avandia, Actose) to lower blood insulin levels and severe restriction of foods with added sugar and flour. Over the last 20 years, doctors have treated hirsutism with birth control pills and spironolactone, a drug to block male hormones. Now they also have leuprolide (brand name Leupron) which stops the brain from stimulating the ovaries to produce testosterone (1) and Proscar, the drug to shrink a man's enlarged prostate.(2,3,4) The most effective treatment may be the anti-male hormone, flutamide (brand name Eulexin) to stop the body from responding to testosterone (5,6,7,8,9,10,12). Doctors can measure the benefit of these drugs by drawing PSA blood tests in women (11)
