Endocrinology and Turner's Syndrome What is Turner's Syndrome? Turner's Syndrome is a relatively common disease in females, affecting many body systems. In this genetic disorder, the absence of all or part of one sex chromosome is associated with short stature and failure to mature sexually, although there is a great variability in the severity of the abnormalities. Other problems can include learning difficulties, skeletal abnormalities, hearing loss, liver dysfunction, heart and kidney abnormalities, infertility, and thyroid dysfunction. Who is affected? Approximately one in every 2,000 female babies has Turner's Syndrome. Among spontaneously aborted fetuses, the frequency is much higher. The condition first may be recognized in the newborn baby, in the child, in the adolescent, or rarely in the adult. Those affected require special medical surveillance throughout life. What causes Turner's Syndrome? Turner's Syndrome is characterized by a deficiency in the amount of genetic material on the X-chromosome, one of the two sex-chromosomes. A careful examination of genetic material, usually in a blood sample, can confirm the diagnosis. The type and amount of missing genetic material influence which specific organ abnormalities are found, as well as the person's potential for growth. More than 90 percent of the females with Turner's Syndrome have under-developed ovaries.

Women with Turner's Syndrome lack one or part of one of the so-called X chromosomes, i.e. sex-chromosomes, that carry genes for conditions relating to the development of ovaries, sex-hormone production, and physical development in general. This relationship leads to faulty development of the ovaries and sex-hormone production, and due to this these women usually cannot get pregnant. Menstruation usually also fails to come, and usually they do not have normal development of breasts and pubic hair. Estrogen treatment should be given from the age of about 12, starting with very small doses. A booklet about estrogen treatment of girls and adult women with Turner's Syndrome can be read or downloaded As there are height determination genes in the X chromosomes, the lacking X chromosome leads to decreased growth and final height. The average height of Turner women is 147 cms, varying between 135 and 163 cms. This is 20 cms below the height of women with normal chromosomes. The reduced height is a handicap, but growth velocity and final height can be increased with growth hormone treatment from the age of 7. Approximately 40 % of the Turner girls have a so-called bed-neck, and in a number of cases it may be necessary to do surgery for this. 10 % have a stricture of the main artery, and approximately 12 % have other less serious heart-anomalies. With strictures of the main artery surgery is often necessary, but this is done with good results.

Genetics Index Glossary XO Gonadal Dysgenesis (Turner's Syndrome) X chromosome monosomy has been described in many species, including humans mice and horses . It is often called Turner's syndrome, or described pathologically as gonadal dysgenesis. Individuals with this disorder have a female phenotype, but invariably have abnormalities in reproductive function. Human Turner's Syndrome Turner's syndrome is the most common sex chromosome abnormality of human females. Its incidence in newborns is approximately 1 in 2,500. However, roughly 10% of spontaneously aborted fetuses have this disorder and the incidence has been estimated as 0.8% in zygotes, making it possibly the most common chromosomal disorder in man. It thus appears that only about 1% of human 45,X xygotes survive to term, and those individuals typically have a rather characteristic set of somatic abnormalities. In roughly two-thirds of cases, the single X chromosome is of maternal origin. Both the embryonic lethality and Turner's phenotype are thought to be due to haploinsufficiency - monosomy of genes common to both the X and Y chromosomes. These genes are among those on the X chromosome that escape X-inactivation and hence, would be expected to be expressed in normal females from both X chromosomes. Interestingly, at least two X-linked genes known to not undergo inactivation in man do undergo inactivation in mice - apparently, a haploid dose of these gene products is sufficient for normality in mice (see below).

Turner's Syndrome: A Case Study by: Carolyn Duda Turner's Syndrome is a genetic disorder that affects about one in every 2000 females born. A female with a normal genetic make-up has two X-chromosomes in each of her cells, one which she received from her father and one which she received from her mother. On the other hand, the typical female with Turner's Syndrome has only one X-chromosome in each of her cells due to a nondisjunction event during meiosis of her parents' gametes. There are several variations on this theme as other similar chromosome anomalies occur in a smaller percentage of females with Turner's Syndrome. Depending on the degree of nondisjunction of her parents' gametes, the genetic make-up of a Turner's patient can vary. As discussed above, the majority of patients have only one X-chromosome in all of their cells and thus they are karyotyped 45,XO. The cells in about 15 percent of Turner's patients contain a normal X chromosome plus an X-isochromosome. This isochromosome consists of the two long arms of the X-chromosome but no short distal arm. These individuals are karyotyped 46,XXp- (Nora and Fraser, 1989). Lyon hypothesized that early in the development of a normal female embryo, random inactivation of one of the two X-chromosomes in each cell occurs. This allows the female to have the same amount of X-chromosome material as the average male has. Recent studies have shown that there are genes on the X-chromosome which escape this inactivation. While there are genes located in various regions of the X-chromosome which escape inactivation (Davies, 1991), many of these genes are located on the short distal arm (Brown and Willard, 1990). In this way, the normal female has functioning genes from one complete X-chromosome plus functioning genes from the still active short distal arm of the mostly inactivated X-chromosome. On the other hand, females with the X-chromosome without this short distal arm lack the genes which would normally have remained active. As a result, a female with this X-isochromosome displays the same phenotype as the typical Turner's female who has only one X-chromosome.
