    Understanding TMJ The normal anatomy of the TMJ consists of three major components.
    • A. The glenoid articulating fossa B. The mandibular condyle C. The articular disc.
    Temporomandibular Joint problems (TMJ) are diversely classified as intracapsular and extracapsular conditions. Intracapsular being in the joint structures like the mandibular condyle, the articular disc and the glenoid fossa. Extracapsular being in the muscles, tendons and ligaments surrounding and connecting to the TM joints and mandible itself. INTRACAPSULAR These disorders are featured by structural alterations of the temporomandibular joint which interfere with the form and function and frequently produce pain. They are made up of four groups of disorders.
    • Developmental Disorders: These problems occur as a result of abnormal development of the joint. Diseases: These consist of degenerative arthritis, rheumatoid arthritis and neoplasms. Injuries: These occur when trauma is induced to the jaw directly or indirectly like in Whiplash. Internal derangements: These occur when the physiologic relationship of the condyle, disc, and fossa have been lost. Internal problems constitute the vast majority of this group.

    TMJ TREATMENT MODALITIES Early treatment is very important and cannot be overstated. TMJ problems can advance in stages and grow progressively worse. Thus, a minor problem now could become a major source of pain in the future. Additionally, chances of successful therapy increase substantially the earlier treatment is accomplished. After diagnosing the problem, a variety of treatments may be recommended depending on the extent of correction needed. Initial phases of treatment are aimed to eliminate the TMJ pain resulting from the actual problem. In the second stage, the actual treatment to correct the problem is implemented. Medicines/Muscle Stimulation/Ultrasound These may be prescribed to alleviate the pain before and during the actual treatment. Trigger Point Injections This is the injection of anesthetic into muscle to eliminate muscle spasm. Splint/Jaw Repositioner Appliances This is a mouth appliance that is custom made to fit between the patient's upper and lower teeth. It is not only used in diagnosis, but they are also used as treatment to temporarily eliminate the bite disharmony and resulting pain. Then once we have achieved a state of muscle relaxation and an acceptable level of patient comfort then we can determine which forms of permanent treatment are necessary to maintain long term comfort. Occlusal Equilibration The tops of the teeth are reshaped in this procedure. This is done to alleviate pressure on individual teeth. The reshaping procedure has been found to be the solution for many conditions which cause discomfort in the head, neck, and shoulders and the breakdown of dental structures. This can be likened to a pebble in your shoe and your foot adapts to that pebble and you may limp. As with a tooth too high for comfort, the jaw also adapts to the uncomfortable tooth fit but develops muscle pain. During equilibration the high tooth is reshaped for a better fit and resulting muscle relaxation.

    What is the point of this website? During the thirty years I have practiced as a restorative dentist, I have been fascinated by the habit of some individuals to grind and clench their teeth. This phenomenon is peculiar for several reasons:
    • The flattened teeth of our ancestors indicate that compression and grinding of teeth has existed for centuries. Even today, it is of epidemic proportions because in most cases it is not diagnosed, not well understood, and consequently not treated properly.
      Thirty percent of the affected patients are not aware that they are damaging themselves, as this habit tends to be subconscious.
      Compression and grinding of one's teeth simultaneously applies undue pressure to the bone supporting the teeth and to the temporomandibular joint (TMJ), where the mandible (lower jaw) hinges with the skull. In my opinion, this is responsible for the majority of problems related to this joint.
      The physical signs of compression are not recognized by the majority of the dental profession. A patient may be chronically damaging him or herself by compressing and grinding, but may nonetheless be quite comfortable. If this is the case, the signs of damage should be recognized when the patient has his or her teeth cleaned. The problem is that sixty-five percent of the dental profession does not recognize these signs and does not understand the implications.

    MPD vs. Intra-articular Disease
    One of the most important concepts to understand is the basic difference between true interarticular disease and the myofascial pain complex of symptoms which the lay press tends to erroneously label as TMJ. These two entities can, and often do, coexist but they are not synonymous. In patients where both are present it is almost always helpful and desireable to treat the MPD first and then address the interarticular pathology. The most notable exception to that treatment sequence is the onset of acute interarticular disease in a previously healthy joint.
    Surgical vs. Non-Surgical Treatment
    For MPD the majority of treatment options are non-surgical however, some consider brisement used for trismus secondary to immobilization syndrome a surgical procedure. Warmth, massage, physical therapy including range of motion exercises, non-steroidal anti-inflammatories reduction in parafunctional activity and patient education form the bulk of the treatment protocols. Many practicioners believe in the construction of intraoral "splints" of a variety of types to treat parafunction. Some purport to treat interarticular disease with these devices. The ability of such devices to treat interarticular disease is not supported by the scientific and orthopedic literature. Their efficacy in the treatment of muscle spasm/myofascial pain is perhaps a little higher but often no better than what can be obtained through the utilization of over the counter rubber mouthpieces such as are provided for high school athletes in contact sports.
