Joan E. (Larmore) Simms Education and Military Service I am a Vietnam Era U. S. Air Force Veteran who served from 1962 to 1966 under the maiden name of Larmore. After completing Basic Training at Lackland (January 1963), I was stationed with the 3201st Air Base Wing WAF Squadron at Eglin AFB, Florida. In 1965 I was selected as the Fire Prevention Queen for the base. My last assignment before receiving a medical discharge was in Air Operations at Eglin. It is the Temporo-Mandibular Joint (TMJ), the joint where the mandible (the lower jaw) joins the temporal bone of the skull, immediately in front of the ear on each side of your head. A small disc of cartilage separates the bones, much like in the knee joint, so that the mandible may slide easily; each time you chew you move it. But you also move it every time you talk and each time you swallow (every three minutes or so). It is, therefore, one of the most frequently used of all joints of the body and one of the most complex. You can locate this joint by putting your finger on the triangular structure in front of your ear. Then move your finger just slightly forward and press firmly while you open your jaw all the way and shut it. The motion you feel is the TMJ. You can also feel the joint motion in your ear canal. These maneuvers can cause considerable discomfort to a patient who is having TMJ trouble, and physicians use these maneuvers with patients for diagnosis.

What are the most important symptoms I should be concerned about? Pain, and or Jaw "locking" episodes. What is a "locking" episode? A "locking" episode can occur during opening or closing movement. What happens is that the patient experiences an interruption of jaw movement - a "catch" or a "stop", and in order to complete the movement must jiggle, or somehow, self manipulate the jaw. Why does it happen? Referring back to my anatomy lesson, and in the simplest of terms, what is happening within the Joint is that the Articular Disk which rides on top of the Condyle Head is getting stuck in the wrong place and is preventing the Condyle Head from moving. If I can jiggle my jaw and reduce the dislocation, why should I be concerned? Because each time it happens more damage is occuring to the tissues in the Joint, and the tissues controlling the Articular Disk. As a consequence there is the risk that if the problem is not addressed by appropriate treatment, one day you will be unable to reduce the dislocation yourself, and you will require an emergency visit to a TMJ practitioner, if one is available in your community, or an oral surgeon. In severe cases, reduction can only be accomplished under general anesthesia.
