Sinusitis
Sinuses are a part of the upper respiratory system. The sinuses function to lighten the skull and improve the quality of the voice. Adults and older children have four groups of sinus cavities located within the bones surrounding the nose. Very young children, however, have small sinus passages rather than fully formed sinuses. To work properly, the sinuses need adequate mucus drainage and a functioning immune system to fight off infections.
What Is Sinusitis?
Sinusitis is an inflammation of the mucous membranes that line the sinus cavities. This inflammation causes the mucous glands in the sinuses to secrete more mucus. A significant amount of inflammation and mucus can lead to obstruction of the sinuses, preventing drainage. The common cold, allergic rhinitis or hay fever, chronic rhinitis, deviated septum, and nasal
polyps can cause swelling of the nasal and sinus passages and lead to sinusitis. Also, smoking or the breathing in of second-hand smoke interferes with normal functioning of the sinuses, leading to sinusitis. Diseases such as cystic fibrosis, bronchiectasis, immune deficiencies and immotile cilia syndrome can also cause sinusitis. Very young children may be prone to developing sinusitis because their smaller sinus passages become obstructed more easily than those of adults.

Introduction to Sinusitis
Sinusitis affects about 14% of the population. Its direct medical costs amount to billions of dollars annually. Indirectly, its cost is greatly magnified by the staggering amount of lost hours of work or school. Current methods of cost analysis indicate that sinusitis usurps nearly 1% or the entire Gross National Product. Clinically, it is defined by physicians as a condition which is manifested by an inflammation involving the muscous membranes of the nasal cavity and the paranasal sinuses. The secretions found in these passages are related to the pathologic changes in both the surrounding bone and tissue. Sinusitis is classified as:
Acute Sinusitis
Acute Sinusitis is sudden in onset and lasts up to 4 weeks. The symptoms resolve completely and, once the disease has been treated, antibiotics are no longer needed. A strong history consistent with acute Sinusitis includes two or more major factors or one major and two minor factors. However, the finding of nasal purulence is a strong indicator of an accurate diagnosis. A suggestive history for which acute Sinusitis should be included in the differential diagnosis includes one major factor or two or more minor factors. In the absence of other nasal factors, fever or pain alone does not constitute a strong history. Severe, prolonged, or worsening infections may be associated with a nonviral element. Factors suggesting acute bacterial Sinusitis are the worsening of symptoms after 5 days, the persistence of symptoms for more than 10 days, or the presence of symptoms out of proportion to those typically associated with a viral (upper respiratory) infection.

    The sinuses are air spaces found in the facial bones that surround the nose, including areas within the cheek bones and in the bones of the forehead above the eyes. When these sinus spaces become inflamed or infected, the infection is called sinusitis. Sinusitis can be either acute (symptoms present for less than 1 month), subacute (symptoms for 1 to 4 months), or chronic (symptoms for more than 4 months). Acute sinusitis may cause different symptoms in different age groups. In adults and older children, for example, symptoms include: a stuffy nose; a discharge from the nose that is yellow, green, bad-smelling or tinged with blood; redness inside the nose; swelling or dull pain around the eyes; tenderness in the area of the cheeks or around the eyes; cheek pain that may be mistaken for a toothache; a feeling of "pressure" in the head; a morning headache; a headache that gets worse when the child bends forward; and bad breath. Sometimes there may also be a dry cough or a low-grade fever, and the child may have trouble sleeping. Stomach upset with vomiting of mucus may be present. Younger children may have any of the symptoms listed above, but sometimes their complaints are less specific. They may appear to have a "

Saini, M.D., originally from Ohio, graduated from the University of Miami School of Medicine in 1992. At Thomas Jefferson University Hospital in Philadelphia, Dr. Saini completed a three year residency program in Pediatrics. He then pursued an Allergy-Immunology fellowship at Northwestern University, Chicago, where he trained in both adult and pediatric allergy. In 1998, Dr. Saini completed an additional fellowship in Clinical Laboratory Immunology at the University of California, Irvine. Dr. Saini is Assistant Clinical Professor of Pediatrics, Division of Allergy-Immunology at the University of California, Irvine. Dr. Saini participates in numerous medical associations, and has volunteered at asthma camps sponsored by the American Lung Association. This clinical practice guideline formulates recommendations for health care providers regarding the diagnosis, evaluation, and treatment of children, ages 1 to 21 years, with uncomplicated acute, subacute, and recurrent acute bacterial sinusitis. It was developed through a comprehensive search and analysis of the medical literature. Expert consensus opinion was used to enhance or formulate recommendations where data were insufficient. A subcommittee, composed of pediatricians with expertise in infectious disease, allergy, epidemiology, family practice, and pediatric practice, supplemented with an otolaryngologist and radiologist, were selected to formulate the practice parameter. Several other groups (including members of the American College of Emergency Physicians, American Academy of Otolaryngology-Head and Neck Surgery, American Academy of Asthma, Allergy and Immunology, as well as numerous national committees and sections of the American Academy of Pediatrics) have reviewed and revised the guideline. Three specific issues were considered: 1) evidence for the efficacy of various antibiotics in children; 2) evidence for the efficacy of various ancillary, nonantibiotic regimens; and 3) the diagnostic accuracy and concordance of clinical symptoms, radiography (and other imaging methods), and sinus aspiration.

    While Dr. James Bishoff of Montana was at the hospital visiting patients he happened to notice a flyer announcing an Immune System Disorders Seminar being held that same week in his area.
    Always wanting to stay up to date on healing therapies for immune system disorders and autoimmune disorders such as Fibromyalgia, Chronic Fatigue Syndrome, Epstein Barr, Lupus, etc. Dr. Bishoff decided to attend.     Tired Of Being Sick You see, although Dr. Bishoff is a medical doctor, he was always sick. In fact, the year he went to the immune system disorder seminar he had been sick 5 times in 4 months. All the antibiotics he took helped, but shortly after - he'd get sick again. He'd get a flu shot and he'd still get the flu.
    His wife was sick as well. She had continuous bouts with chronic bladder infections and took antibiotics as well without much success. From the bladder infections and antibiotics she got yeast infections. At the time Dr. Bischoff attended the immune system disorders seminar she was dealing with a severe yeast infection that had already lasted 4 months.
