The first step in diagnosing Parkinson's disease is to make certain the symptoms are not being caused by something else. Since there are no specific tests that confirm beyond a doubt that a person has this disease, doctors often base their conclusion on a clinical examination and tests such as a CAT (Computed Axial Tomography) scan to eliminate other reasons for patient's symptoms. Newer tools, SPECT (Single Photon Emission Computed Topography) and PET (Position Emission Tomography) are sometimes used to diagnose Parkinson's disease. Early Stages The following symptoms may or may not be seen in the early stages of the disease.
  • The shaking of the hands, arms, legs or feet while the patient is resting. This shaking is often the symptom that prompts the person to visit the doctor. It may be more noticeable on one side of the body, and it may affect the hands more than the feet. The shaking usually stops, however, as soon as the patient uses his or her muscles.
  • Slow movement or a brief, temporary delay in movement

There are several drugs which can be used to treat the symptoms of Parkinson's disease, and one of the challenges for doctors is deciding when each patient should begin taking drugs, which drug(s) would benefit them the most, and when to change the dose(s) of these medicines. If you are currently seeing a doctor and have been prescribed any medicines, like MIRAPEX Tablets , it is important to follow the dosing regimen prescribed by your physician. Replacement Drugs
The most widely recognized drug for the treatment of Parkinson's disease is levodopa, a medication that is converted into dopamine when it crosses into the brain. Levodopa reduces the symptoms of this disease because dopamine is the chemical that is necessary for human muscles to function normally. It is called a replacement drug because it replaces dopamine. For most patients with Parkinson's disease, levodopa provides significant improvements in their symptoms. However, the therapeutic benefits of this drug usually last for only a limited period of time. After about 3 to 5 years of treatment, levodopa becomes less effective in reducing symptoms in most patients. At times the patient can move around easily or with slight tremor and rigidity, and at other times the patient has difficulty with movement, as if the medicine is "wearing off." For this reason, doctors try to keep the dose of levodopa as low as possible or may not begin treatment with levodopa until the symptoms can no longer be managed by other means.

Introduction
Parkinson's Disease
is a slowly progressive disorder of the central nervous system that affects movement, muscle control and balance. Although the exact cause of Parkinson's Disease is unknown, research has concentrated on genetics, environmental toxins, endogenous toxins and viral infection.
In Parkinson's, cells are destroyed in part of the brain stem - the substantia nigra , which sends out fibers to the corpus stratia , gray and white bands of tissue in both sides of the brain. Cells there release dopamine , one of three major neurotransmitters (chemical messengers) which help the body respond to stress. By the time symptoms develop, patients have lost 80 to 90 percent of their dopamine-producing cells.
Symptoms include tremors, slowed movement and postural instability. Other features include rigidity, flexed posture, freezing phenomenon and loss of postural reflexes. Patients can experience depression, sleep disturbances, dizziness and problems with speech, swallowing and sexual functioning.

James Parkinson established the condition as a clinical entity and his essay continues to be recognised through the world as the classic description of the condition. Parkinson's disease is a progressive neurological disorder affecting learned voluntary movements such as walking, talking, writing and swallowing. There are three main symptoms: tremor. rigidity and bradykinesia (slowness of movement). However not everyone will experience all three symptoms. Parkinson's disease results from the loss of the chemical messenger, dopamine, within the centre of the movement in the mid­brain. The cause is as yet unidentified and there is no known cure. Parkinson's disease is not inherited nor is it contagious. The incidence of Parkinson's disease is approximately 1:1000 in the general population, rising to 1:100 over the age of 65 and 1:50 over the age of 80. A little recognised fact is that 1:7 people diagnosed are aged under 40. There are more than 120,000 people with Parkinson's disease in the United Kingdom today and it is common in all parts of the world. The main treatment for Parkinson's disease is drug therapy. Because no two people with Parkinson's disease are exactly alike, the drug treatment has to be tailored to the individual and the timing of medication is as important as the dosage. With the optimum drug treatment, life expectancy for those with Parkinson's disease is normal. Management plays a very important part in the car of people with Parkinson's disease and professions such as physiotherapy, speech therapy, and occupational therapy have an important role to play.

A brain disease which, like Alzheimer's is uncomfortably common as we age. It's Parkinson's. And while there are drugs to treat it they are far from ideal. So researchers have been looking at drugs which might prevent the diseases progress and even forms of brain surgery including transplantation and a technique called 'deep brain stimulation'. One of the people who's been involved with all three is Warren Olanow, who's Professor of Neurology at Mount Sinai School of Medicine in New York. Professor Olanow: Parkinson's is a disease that is due to degeneration of nerve cells in the centre of the brain called the Substantia Nigra. When these cells degenerate patients lose a chemical called dopamine. And this results in a number of signs and symptoms which include tremor, rigidity or stiffness, bradykinesia or slowness, and difficulty with walking, tendency to fall, postural instabilities etc. Norman Swan: Has it also recently been shown that it's actually more widespread than the substantia nigra, large parts of the brain are affected and it can affect thinking ability as well?

Summary: Parkinsons Disease Patients with Dementia
Study Description
A 24 week Multicentre, double-blind placebo controlled study of the efficacy, tolerability and safety of Exelon (Rivastigmine) in patients with Parkinson's Disease Dementia. (PDD) There are currently no licensed treatments for PDD and this study is investigating the use of a treatment licensed for Alzheimers Disease in this patient population. Study Criteria
  • Patients have to be over 50 years old and have a diagnosis of Parkinsons Disease (PD).
  • There must be 2 years between the diagnosis of PD and the onset of symptoms of Dementia (Mini Mental State Score 10 - 24)
  • Patients need a caregiver who visits them a minimum of 3 times per week.
  • Patients on other Cholinesterase inhibitors (eg Aricept, Galantamine, Tacrine) will be excluded.

The Institute is an independent, not-for-profit organization conducting patient care and research activities in the neurological specialty area of movement disorders. Our mission is to find the cause and cure for these disorders, to provide the best available medical care to patients with movement disorders, to investigate better treatment and diagnostic tools, and to develop prevention strategies. Programs at the Institute are divided into three main areas: 1) patient care, 2) clinical research and 3) basic research. Having all these programs under one roof fosters close collaboration among physicians, patients and caregivers, and clinical and basic researchers, greatly enhancing productivity and creativity. Our "one-stop shop" approach also allows for a very focused and integrated attack on Parkinson's disease. In the Movement Disorders Clinic, four specialists see approximately 1,500 patients each year for Parkinson's disease, essential tremor, Huntington's disease, Tourette syndrome, progressive supranuclear palsy and other movement disorders. Other services include physical therapy, gait and balance classes, monthly seminars, a resource library, and education and support for patients and caregivers. Patients often come to The Parkinson's Institute from other areas of the United States and abroad for consultation and diagnosis. Dr. James Tetrud is Medical Director of the Institute's Movement Disorders Treatment Center. Dr. Tetrud has recently been joined by Steve Sowiski, the new Director of the Clinic and Clinical Programs and Tracy Stewart, RN, the Clinic and Clinical Trials Nurse Manager.

    Scientists looking for a cure for Parkinsons Disease have found that Ecstasy may hold some of the answers.
    A study was conducted at Manchester University where 'E' was given to Monkeys with the disease. The results took everyone by surprise! Each of the Monkeys had been given L-dopa on a daily basis (a drug given to treat the disease). A side effect of this drug is the uncontrollable movement to the subject's limbs. However, after receiving a dose of ecstasy, the monkey's uncontrollable movements were either stopped all together or drastically reduced by up to 85%. Scientists now believe their findings could help develop a new treatment for Parkinson's disease based on ecstasy.
