Antibiotic impregnated acrylic bone cement is well accepted in orthopedics. Several brands containing the aminoglycoside antibiotic gentamicin are marketed. To obtain information on the elution profile of the included antibiotic from the surgical cement, we studied the in vitro release of gentamicin from two marketed brands: Palacos® and Palamed®. Two experimental arrangements were used to investigated the release of gentamicin from the bone cement. The elution of gentamicin was tracked both in a non-flowing (non-dynamic time-elapsed elution) setting and by the use of a clearly defined USP XXIV flow-through cell apparatus (#4; pg1945), through which an elution medium was pumped at a constant speed.

Paget's Disease of the Bone What is Paget's disease of the bone?
Paget's disease of the bone is a chronic bone disorder in which bones become enlarged and deformed. Bone may become dense, but fragile, because of excessive breakdown and deformation of bone. The disease affects both genders, is rarely found in people under the age of 40, and occurs in up to 3 percent of the US population. What causes Paget's disease of the bone? The exact cause of Paget's disease of the bone is unknown, but it is suggested to be due to a slow viral infection of bone and may include a heredity factory. What are the symptoms of Paget's disease of the bone? The following are the most common symptoms for Paget's disease of the bone. However, each individual may experience symptoms differently. Symptoms may include:
  • pain in the affected area deformity of bone in the affected area susceptibility to fractures in the affected area headache (if the affected area is the skull) hearing loss (if the affected area is the skull)
The symptoms of Paget's disease of the bone may resemble other bone disorders or medical problems. Always consult your physician for a diagnosis.

Fractures What is a fracture?
A fracture is a partial or complete break in the bone. When a fracture occurs, it is classified as either open or closed:
  • open fracture - the bone exits and is visible through the skin, or a deep wound that exposes the bone through the skin. closed fracture - the bone is broken, but the skin is intact.
Fractures have a variety of names. Below is a listing of the common types that may occur:
  • transverse - the break is in a straight line across the bone. spiral - the break spirals around the bone; common in a twisting injury. oblique - diagonal break across the bone. compression - the bone is crushed, causing the broken bone to be wider or flatter in appearance. comminuted - the break is in three or more pieces and fragments are present at the fracture site.

A New Resorbable Bone Void Filler in Trauma: Early Clinical Experience and Histologic Evaluation
A variety of synthetic replacement materials for cancellous bone grafts are currently available for use. These materials are made from calcium salts such as calcium sulfate, calcium carbonate, and calcium phosphate, or from bioactive glasses containing calcium oxide. Synthetic bone replacements can be removed from defect sites by physical and biologic mechanisms. Within a healing bone defect, particles and scaffolds of calcium sulfate and calcium carbonate are generally dissolved before new bone can grow into the defect. Dense HA is virtually insoluble and TCP dissolves slowly and is more fully resorbed due to its porosity. -Figure 1: Radiographs from bone donor site of Case 1. One-month (A) and 7-month (B) anteroposterior views showing radiopacity of ultraporous implant diminishing with time in the proximal tibia of bone harvest site indicating consolidation of new bone into the defect site as the implant is being resorbed. Existing synthetic graft materials possess intrinsic high biocompatibility within a narrow range, but physical qualities vary considerably. The space between individual granules or pellets provides the sole communication between the internal volume of a defect site packed with dense synthetic HA granules or calcium sulfate pellets, and surrounding biologic tissue. Cast calcium sulfate obliterates the volume of the defect site, inhibiting tissue penetration.

Dear Colleague: More than 450,000 bone grafts are performed each year in the United States. While allograft and autograft are the most commonly used graft materials, recent advances in technology have made the use of synthetic skeletal substitute materials a comparable alternative. Although synthetic materials are currently used in only 10% of orthopedic procedures worldwide, the progress and evaluation of this material show the prospect of rapid evolution. Through preclinical and clinical trials, all of the synthetic materials available are being monitored. A variety of biosynthetic and synthetic materials are available to orthopedics surgeons, including demineralized bone matrix, collagen, ceramics, cements, and polymers, such as silicone and some acrylics. These materials can serve as a structure on which new bone can grow. Many of these materials then dissolve over time, leaving only new bone behind. Internationally recognized expert in bone healing and total joint replacement, recently named as the first recipient to The Albert and Margaret Hofmann Endowed Chair in Orthopedic Research, Department of Orthopedics. Holds positions as a research professor in orthopedics, bioengineering, and biology and is a senior VA Research Scientist and Co-Director of the VA Bone and Joint Research Lab, which is a collaborative research program with the VA Salt Lake City Health Care System Research and Development Program. In addition, he is a Research Career Scientist at the Department of Veterans Affairs (VA) Salt Lake City Health Care System and is mainly supported by the Department of Veterans Affairs (VA), Health Administration Research and Development Merit Review Program. Publications include over 77 peer reviewed manuscripts on bone and total joint replacement related topics and has been a guest lecturer on these topics all over the world. He has received numerous academic awards. Received doctorate, as a Rotary International Scholar, from the University of Western Australia. Has been on the faculty at the University of Southern California and Arizona State prior to taking a position at the Department of Orthopedics. Research interests are biomaterials, bone, total joint replacement, articular cartilage and electron microscopy, including pioneering efforts in backscattered electron imaging. Does my child have weak bones?
On one hand, younger bones are more flexible and resistant to injury than older, brittle bones. Young bones can stand more stress due to the plasticity and porous nature of growing bone. But occasionally the bone may fracture, or break. Even multiple fractures are not necessarily a sign of weak bones. Fractures such as in the wrist, forearm or elbow are most common of all the fractures we see in children because they attempt to break their fall with their hands and wrists. Very few children have abnormally weak bones. Some of these children may have Osteogenisis Imperfecta, also known as Brittle Bone Disease. Many of these children have a fracture in the first year or two of life, and low stress injuries may cause fracture. A new test called bone densiometry is used of to determine risk of fracture.
