What is Conduct Disorder?

According to DSM-IV criteria, conduct disorder may be diagnosed when a child seriously misbehaves with aggressive or nonaggressive behaviors against people, animals or property that may be characterized as belligerent, destructive, threatening, physically cruel, deceitful, disobedient, or dishonest. This may include stealing, intentional injury, and forced sexual activity. Keep in mind that this behavior disorder consists of a pattern of severe, repetitive acting-out behavior and not of an isolated incident here and there. As stated, conduct disorder is a repetitive and persistent pattern of behavior in which the basic rights of others, or major rules and values of society are violated, as shown by the presence of three (or more) of the following behavior patterns in the past 12 months, with at least one behavior pattern present in the past six months: Aggression to people and animals:
  • Often bullies, threatens, or intimidates others.
  • 6. Oppositional Defiant Disorder

As with Conduct Disorder the main features observed are persistent disobedience and opposition to authority figures (such as parents, teachers or other adults). However the basic rights of others are still respected and age appropriate societal rules and behaviour are not violated. Characterised by negative, hostile and defiant behaviour, generally strongest in the home. Before puberty the disorder is more frequently found in males than females, after puberty the ratio equals out. Outward behaviour includes irritability, temper outbursts, frustration and intolerance. The child’s self-esteem is usually low (although child may project an image of “toughness”). Additional symptoms of anxiety and depression are also common.
Only to be considered Oppositional Defiant Disorder if at least of the following behaviours occurred over a period of at least 6 months, in home or school

    Oppositional Defiant Disorder

    Diagnostic Guidelines

The essential feature of this disorder is a pattern of persistently negativistic, hostile, defiant, provocative, and disruptive behaviour, which is clearly outside the normal range of behaviour for a child of the same age in the same sociocultural context, and which does not include the more serious violations of the rights of others as reflected in the aggressive and dissocial behaviour specified for categories F91.0 and F91.2. Children with this disorder tend frequently and actively to defy adult requests or rules and deliberately to annoy other people. Usually they tend to be angry, resentful, and easily annoyed by other people whom they blame for their own mistakes or difficulties. They generally have a low frustration tolerance and readily lose their temper. Typically, their defiance has a provocative quality, so that they initiate confrontations and generally exhibit excessive levels of rudeness, uncooperativeness, and resistance to authority. Frequently, this behaviour is most evident in interactions with adults or peers whom the child knows well, and signs of the disorder may not be evident during a clinical interview.

C HILDREN W ITH O PPOSITIONAL D EFIANT D ISORDER No. 72 All children are oppositional from time to time, particularly when tired, hungry, stressed or upset. They may argue, talk back, disobey, and defy parents, teachers, and other adults. Oppositional behavior is often a normal part of development for two to three year olds and early adolescents. However, openly uncooperative and hostile behavior becomes a serious concern when it is so frequent and consistent that it stands out when compared with other children of the same age and developmental level and when it affects the child's social, family, and academic life. In children with Oppositional Defiant Disorder (ODD), there is an ongoing pattern of uncooperative, defiant, and hostile behavior toward authority figures that seriously interferes with the youngster's day to day functioning. Symptoms of ODD may include:
    • frequent temper tantrums
    • excessive arguing with adults
    • active defiance and refusal to comply with adult requests and rules
    • deliberate attempts to annoy or upset people
    • blaming others for his or her mistakes or misbehavior
    • often being touchy or easily annoyed by others
    • frequent anger and resentment
    • mean and hateful talking when upset
    • seeking revenge
The symptoms are usually seen in multiple settings, but may be more noticeable at home or at school. Five to fifteen percent of all school-age children have ODD. The causes of ODD are unknown, but many parents report that their child with ODD was more rigid and demanding than the child's siblings from an early age. Biological and environmental factors may have a role.

Oppositional Defiant Disorder (ODD) What is it? ODD is a psychiatric disorder that is characterized by two different sets of problems. These are aggressiveness and a tendency to purposefully bother and irritate others. It is often the reason that people seek treatment. When ODD is present with ADHD, depression, tourette's, anxiety disorders, or other neuropsychiatric disorders, it makes life with that child far more difficult. For Example, ADHD plus ODD is much worse than ADHD alone, often enough to make people seek treatment. The criteria for ODD are: A pattern of negativistic, hostile, and defiant behavior lasting at least six months during which four or more of the following are present:
    • 1. Often loses temper 2. often argues with adults 3. often actively defies or refuses to comply with adults' requests or rules 4. often deliberately annoys people 5. often blames others for his or her mistakes or misbehavior 6. is often touchy or easily annoyed by others 7. is often angry and resentful 8. is often spiteful and vindictive
The disturbance in behavior causes clinically significant impairment in social, academic, or occupational functioning.
