Chronic Diseases in the United States
  • Chronic diseases account for 70 percent of all deaths in the United States and the medical care costs account for more than 60 percent of the nation's medical care costs. More than 90 million Americans live with chronic illnesses. Chronic diseases are long-term illnesses that are rarely cured completely. Most chronic diseases can be prevented or improved by avoiding risky behaviors such as smoking and poor nutrition, or by increasing physical activity and obtaining life-saving screening services. Asthma affects more than 15 million Americans. The projected cost of asthma in this country for the year 2000 was $14.5 billion.

Internet address: lisa_guion@ncsu.edu The North Carolina Cooperative Extension Service 4-H Expanded Food and Nutrition Program (4-H EFNEP), in collaboration with the Out For Lunch program, funded nine counties to implement youth nutrition programs during the summer of 1997. These experiential programs were designed to provide limited resource youth with information on the fundamentals of proper nutrition, and contribute to their personal development and well-being. A total of 1,469 young people in Alamance, Ashe, Craven, Gaston, Hoke, Montgomery, Pasquotank, Warren and Wayne counties were provided with solid nutrition education via fun, hands-on demonstration sessions, cooking classes, food science experiments, experiential learning tours, arts and crafts such as vegetable block printing, as well as hours of nutrition education. Fifty teens were trained as peer nutrition educators and conducted a total of 300 hours of education. Collaboration played a large role in the success of all the county programs. Likewise, members of a collaborative effort view each other as partners and are willing to share risks, resources, responsibilities, and rewards. "Collaboration is exchanging information, altering activities, sharing resources, and enhancing the capacity of organizations, for mutual benefit, and to achieve a common purpose" (Himmelman, 1993, p. 2).

Activities of the Nutrition Services Branch promote sound nutrition habits among infants, children and women in their child-bearing years. Branch staff work with county, state and private agencies to improve health status by reducing the incidence of nutritional risk factors, improving pregnancy outcomes, and hastening recovery from illness and injury through the provision of technical assistance, education and supplemental foods. The Branch has five units: Administrative Services Unit: The Administrative Services Unit administers all aspects of the Vendor component of the WIC Program including vendor contracting, training, and surveillance. Unit staff members process contracts and budgets for local WIC Programs. Clinical Services Unit: The Clinical Services Unit is responsible for planning and implementing nutrition services provided through the Supplemental Nutrition Program for Women, Infants, and Children (WIC), the Child and Adult Care Food Program (CACFP), and the Summer Food Service Program for Children (SFSP); providing leadership in developing nutrition policy, clinical guidance, nutrition education materials, nutrition training and outreach efforts, particularly for women and preschool children; and administering the North Carolina Pediatric and Pregnancy Nutrition Surveillance Systems.

Prenatal women and mothers at all income levels, who choose to breastfeed their infants, receive support through prenatal education and in-home visits after the baby is born. The educational effortgs of the nutrition education staff are enhanced through networking, collaborations and partnerships in the community. The North Carolina Initiative for Healthy Weight in Children and Youth involves both strategic planning activities and pilot interventions. Both components will be guided by an internal project management team consisting of representatives of key public health programs and major partners, and an external advisory committee consisting of members of the North Carolina Task Force on Child and Adolescent Overweight. The Task Force will develop a strategic plan focusing on four general areas: public awareness, primary prevention, secondary prevention (treatment), and surveillance and evaluation. QUALITY STANDARDS FOR SCHOOL-BASED HEALTH CENTERS IN NORTH CAROLINA Background
For many years, public schools have provided important basic health services to students. School- based health centers (SBHCs) represent a relatively new expansion of this concept, offering comprehensive, accessible, primary and preventive physical and mental health services to school-age youth. This evolution is a community based response to the serious health and health-related educational problems faced by many children and their families today. As recently as 1985, fewer than 50 SBHCs existed in the United States. By the fall of 1995, over 600 were in operation. North Carolina currently has 26 SBHCs in eighteen counties. These were developed through local community collaboration, spurred by the vision of parents, educators, and health providers to improve the health and educational status of North Carolina's children and teenagers. The purpose of the Making the Grade in North Carolina project is to increase access to comprehensive health care for school age children and youth of this state through school-based and school linked health centers. Standards have been developed to provide guidance in achieving excellence in care at the centers. These standards will:
