... Bottom Citing publications used in research papers, articles, etc. are not exercises designed to strain the patience of writers. They are locators for material used and should be as precise and succinct as good road directions. The form of the citation is not sacrosanct but is used for uniformity and is a valuable tool for communication. This guide provides examples of the most common examples of government document citations. These examples are based on the Chicago/Turabian standard bibliographic style and not reference-list form. You may need to modify the citation to conform to the manual/form you are required to use. If your publication does not fall into any of these categories, please see the Government Publications Staff for more detailed guidance. The forms of these citations were patterned after the following: Garner, Diane and Diane H. Smith.

HIV Testing In Ontario
August 1998
Can I get tested for HIV without giving my name?
Yes. Many places in Ontario do "anonymous testing" for HIV (Human
Immunodeficiency Virus). Anonymous testing means you don't have to give your name or
address. If you test positive, no one knows except you. No government
or business finds out that you're HIV positive, or even that you were
tested. The back of this pamphlet lists the HIV test sites in Ontario. These are
the only places you can get anonymous testing. Your doctor cannot test
you anonymously.
How can I get tested? Call one of the sites on the list and explain that you want to be tested anonymously. The person you talk to will ask a few questions, such as: Why do you think you might have HIV? Have you had unsafe sex or shared a needle in the last three months? If he or she thinks it makes sense for you to be tested, you will be given an appointment. You must provide a first name, but you can make one up if you want. And you don't need an OHIP number. At your appointment, you are asked for your date of birth and the town

    Today's Urgent Actions
Write to Elected Officials Donate Today Rights. Respect. Responsibility.® Campaign Issues at a Glance Legal Issues and School-Based and School-Linked Health Centers Commonly Asked Questions Through its extensive work with school-based and school-linked health centers (SBHC/SLHCs), Advocates for Youth knows that legal issues greatly concern planners and administrators. Some of the commonly asked questions include:
    • When may adolescents consent to their own treatment? When do parents have the right to see their children's records? When must SBHC/SLHCs notify parents about services provided? If a legal problem arises from operating the health center, who may be liable? What restrictions are imposed by various funding statutes and which supersedes another if a conflict arises?
The answers to these questions are not simple. The laws governing SBHC/SLHC practice are a patchwork of federal, state, and local statutes, regulations, court decisions, constitutional provisions, and restrictions imposed by the clinics' funders. Within this complex framework, each SBHC/SLHC must address the particularly sensitive issues of consent, confidentiality, liability, and funding. This document is not, nor is it intended as, a substitute for consulting with an attorney licensed to practice in the state where the SBHC/SLHC is located. Instead, this is an introduction for staff, advocates, and others to the legal issues facing SBHC/SLHCs, with examples and discussion intended for non-lawyers. Once SBHC/SLHC staff and advocates are familiar with the legal framework, they will be better prepared to consult with local attorneys and officials.
