General Travel Health Center Avoiding Travelers' Diarrhea. Eliot C. Heher, MD Global travel can be extremely enjoyable, but it also creates some health risks for the traveler, including most commonly Travelers' Diarrhea, or TD for short. (This is a condition with many other names, such as Montezuma's Revenge, for example). This column will focus on basic information about TD and the food and water precautions you should follow to avoid this scourge. Next time I'll focus on the treatment of TD. TD is extremely commonstriking 30-70% of travelers to developing countriesand the best evidence is that nearly all cases are caused by infectious agents, such as viruses, bacteria, and protozoa. The two most common symptoms are diarrhea and abdominal cramping. Nausea and vomiting, bloating, malaise, fever and blood in the stool also occur. TD usually strikes quickly, typically during the first week of your trip, but can occur upon return home. Most cases last 3-4 days, untreated, and 90% resolve within a week. According to the CDC, the risk of acquiring TD is highest when you travel to developing countries in Latin America, Africa, the Middle East and Asia. Intermediate risk destinations include countries in Southern Europe and several Caribbean islands. Low-risk destinations include Canada, Northern Europe, Australia, New Zealand, the United States and many of the more developed Caribbean Islands. TD is spread through fecally contaminated food, primarily, and water. Obviously the risk of contamination occurring is highest in places without adequate sanitation facilities. High risk foods include raw and undercooked meats, seafood, raw fruits and vegetables. Tap water, ice and unpasteurized milk and dairy products such as cheese may be associated with increased risk of TD. Where you eat also seems to matter. Highest risk: street vendors: Lowest risk: food prepared in private homes.

June 2002 Caribbean Fact Sheet The Caribbean Sea region serves an important role as a petroleum processing and transshipment area, with several major refineries and independent storage facilities. Trinidad and Tobago is becoming an increasingly significant supplier of liquefied natural gas (LNG) to regional markets, and the United States. Note: information contained in this report is the best available as of June 2002 and can change. GENERAL BACKGROUND The islands of the Caribbean basin, with a total population of approximately 38 million in 2001, are predominantly net energy importers, with the exception of hydrocarbon rich Trinidad and Tobago. Agriculture and natural resource extraction activities continue to constitute the basis of the islands' economies, though the tourism and service sectors are growing. In the larger economies, manufacturing is also important, with oil and gas production stimulating related heavy industries in Trinidad; a more diversified industrial base in Puerto Rico including pharmaceuticals and cement; and the textile industry being important in the Dominican Republic and Jamaica. In 2000, total gross domestic product in the Caribbean basin was approximately $162 billion. OIL AND NATURAL GAS In 2000, the islands of the Caribbean region consumed a combined total of 2.1 quadrillion Btu of energy. Oil is the dominant fuel, accounting for about 90% of total 2000 energy consumption. The Caribbean relies on imported oil for most of its energy needs. Barbados, the Dominican Republic, Haiti, and Jamaica are party to the San Jose Pact, under which Mexico and Venezuela supply crude oil and refined products on favorable terms. Natural gas and hydropower are used in countries that have domestic resources. Natural gas is used most extensively in Trinidad and Tobago, where gas-intensive industries such as steel, fertilizer, and petrochemicals are important to the country's economy. Puerto Rico has begun importing liquefied natural gas (LNG) from Trinidad and Tobago for power generation.

USGS Water Resources of the Caribbean Caribbean District Science Plan 1999 Introduction Severe water rationing has been implemented three times during the 1990's in Puerto Rico. At the same time, significant agricultural losses, valued in the $100's of millions have occurred. A drought in 1994-95 affected more than one million people in the San Juan metropolitan area who endured a regimen of water rationing that lasted for six months in which sections of San Juan had their water-distribution networks shut off on alternate days. During the winter and spring of 1997-98, more than two hundred thousand people in northwestern Puerto Rico experienced severe rationing of public-supplied water as water level in their principal reservoir, Lago Guajataca , fell 10 meters due to rainfall deficits and sustained withdrawals. During the 1990's, annual rainfall accumulation has been the lowest of the 20th century at 7 of the 12 stations with the longest period of record in Puerto Rico, and the second lowest at the remaining five stations. This rainfall deficit may be an indication of a regional drying trend that makes large populations in the West Indies vulnerable to water shortages. The total population of the 24 island nations in the West Indies is 37 million. The critical water shortages that have occurred in Puerto Rico, the third most populous island in the West Indies, provide an example of the impact of water resource shortages on regional population.
